ABSTRACT

Abstract To evaluate the accuracy of complete arch scanning with multiple implant titanium scan bodies using laboratory scanners. A master model of an edentulous maxillary arch with 6 implants was fabricated. Titanium scan bodies were inserted into the model. Three laboratory scanners were used: D2000 (3Shape), Vinyl High Resolution (Smart Optics), and inEos X5 (Dentsply Sirona). The master model was consecutively scanned ten times using dental laboratory scanners (LS) without detaching and repositioning the scan bodies. Linear and angular accuracy between adjacent implants was measured using inspection software (Control X, Geomagic). The accuracy of the complete arch scans was calculated. Implant regions were defined as; parallel (R1: #24-26 and #16-14), angled (R2: #22-24 and #14-12), angled to occlusal plane (R3: #12-22), and cross-arch (R4: #16-26). The effect of LS and implant region on accuracy was compared using two-Way ANOVA (α=0.05). Significant greater linear distortion was noted in R4 (61.2±17.9µm) compared to R1 (23.4±15.5µm) and R2 (26±17.7µm) (p<0.01). Greater linear distortions were noted in R4 with D2000 (0.07±0.016 degrees) and Vinyl High Resolution (0.067±0.02 degrees) than inEos X5 (0.032±0.021 degrees) (p>0.05). Greater mean linear precisions were noted in R1 (9±8µm) and R3 (9.3±8.3µm) than R4 (12.6±10.3µm) (p<0.05). The highest linear precision was noted in D2000 (7.2±7.6µm) (p<0.05). The angular precision of D2000 (0.02±0.015 degrees) was the highest (p<0.01). The angular precisión of R4 (0.036±0.018 degrees) was the lowest (p<0.01). This study revealed that the trueness was affected by the implant region and the precision was affected by both LS and implant region.


Resumen Evaluar la precisión del escaneado de la arcada completa con cuerpos de escaneado de titanio de múltiples implantes utilizando escáneres de laboratorio. Se fabricó un modelo maestro de una arcada maxilar edéntula con 6 implantes. Se insertaron cuerpos de escaneo de titanio en el modelo. Se utilizaron tres escáneres de laboratorio: D2000 (3Shape), Vinyl High Resolution (Smart Optics) e inEos X5 (Dentsply Sirona). El modelo maestro se escaneó consecutivamente diez veces usando escáneres de laboratorio dental (LS) sin separar y reposicionar los cuerpos de escaneo. La precisión lineal y angular entre implantes adyacentes se midió utilizando un software de inspección (Control X, Geomagic). Se calculó la precisión de los escaneos completos del arco. Las regiones del implante se definieron como; paralelo (R1: #24-26 y #16-14), angulado (R2: #22-24 y #14-12), angulado al plano oclusal (R3: #12-22) y cruzado (R4: #16-26). El efecto de LS y la región del implante en la precisión se comparó mediante ANOVA de dos vías (α=0,05). Se observó una distorsión lineal significativamente mayor en R4 (61,2±17,9µm) en comparación con R1 (23,4±15,5µm) y R2 (26 ±17,7µm) (p<0,01). Se observaron mayores distorsiones lineales en R4 con D2000 (0,07±0,016 grados) y vinilo de alta resolución (0,067±0,02 grados) que en inEos X5 (0,032±0,021 grados) (p>0,05). Se observaron precisiones lineales medias mayores en R1 (9±8µm) y R3 (9,3±8,3µm) que en R4 (12,6±10,3µm) (p<0,05). La mayor precisión lineal se observó en D2000 (7,2±7,6 µm) (p<0,05). La precisión angular de D2000 (0,02±0,015 grados) fue la más alta (p<0,01). La precisión angular de R4 (0,036±0,018 grados) fue la más baja (p<0,01). Este estudio reveló que la veracidad se vio afectada por la región del implante y la precisión se vio afectada tanto por LS como por la región del implante.

ABSTRACT Objective: This study aims at identifying anatomical dimensions of dental arches, based on landmarks currently used in the lingual orthodontic technique, and create an archwire form template to be used in orthodontic clinics. Methods: Maxillary and mandibular dental casts of 140 Caucasian individuals with natural and normal occlusion were digitized (3D), and the images were analyzed with Delcam Power ShapeTM 2010 software. The dental arch shapes and sizes were obtained from 14 landmarks selected on the lingual surface of the teeth. Points and segments defined by the software were used to create an archwire form template. Results: Various dental arch patterns were found for both maxilla and mandible. The smallest sizes were found in females, and the largest were found in male subjects. Six categories were defined for each gender, three for the maxilla and three for the mandible (Small, Medium and Large). A template was created with eighteen anatomic lingual archwire designs, nine for the maxilla and nine for the mandible, for both genders. Conclusions: Landmarks evaluated in this study showed dental arch differences between genders. This information enables making orthodontic lingual archwires that are more compatible with the anatomical forms and sizes of the maxilla and mandible. The findings also allowed the creation of a template for an anatomic lingual metallic archwire form to be used in the lingual technique.


RESUMO Objetivo: O presente estudo tem como objetivo encontrar as formas anatômicas e dimensões das arcadas dentárias com base em pontos de referência utilizados na técnica ortodôntica lingual, e criar um diagrama com um maior número de arcos metálicos para serem utilizados na clínica ortodôntica. Métodos: 140 modelos de indivíduos caucasianos com oclusão normal e natural foram digitalizados (3D) e as imagens, analisadas com o software Delcam Power ShapeTM 2010. A determinação das formas e tamanhos das arcadas dentárias foi obtida a partir de 14 pontos selecionados na superfície lingual dos dentes. Outros pontos e segmentos foram utilizados, com o auxílio do software, para definir um diagrama. Resultados: Foram encontrados diferentes tamanhos de arcadas dentárias linguais, tanto para a maxila quanto para a mandíbula. Os menores tamanhos foram os femininos, e os maiores, os masculinos. Definiram-se seis tamanhos para cada sexo, sendo três para a maxila e três para a mandíbula, nomeados como P, M e G. Foi criado um diagrama com dezoito desenhos de arcos linguais anatômicos, nove para a maxila e nove para a mandíbula, para ambos os sexos. Conclusões: A posição dos pontos de referência nesse estudo evidenciou diferenças entre os sexos, o que permitiu a criação de arcos mais compatíveis com as formas e dimensões anatômicas da maxila e mandíbula. A diferença entre os tamanhos das arcadas dentárias linguais possibilitou a criação de um diagrama com formas de arcos metálicos linguais anatômicos para serem utilizados na técnica lingual, para auxiliar o profissional a criar os seus próprios arcos.

Abstract Introduction: Intraoral devices have increasingly assumed a key role in the treatment of obstructive sleep apnea syndrome, but there are limitations to their indication and side effects that result from their continuous use, as well as the use of the continuous positive airway pressure device. Objectives: To evaluate the changes in dental positioning caused by the continuous use of mandibular advancement devices. Methods: A prospective longitudinal study with a sample of 15 patients, with evaluation of complete documentation after a mean time of 6.47 months, assessed changes in dental positioning due to the use of the Twin Block oral device for the treatment of patients with apnea. The following variables were evaluated: overjet, overbite, upper and lower intermolar distances, upper and lower intercanine distances, Little's irregularity index and the incisor mandibular plane angle. An intraclass correlation test was performed and a correlation index > 0.08 was accepted. After verifying the normal sample distribution (Shapiro-Wilks), a parametric test was used (t test), with a significance level set at 5%. Results: There was a decrease in the values of overjet, overbite and Little's irregularity index, whereas there was an increase in the lower intercanine distance and IMPA values. All these variables are influenced, at different levels, by the forward inclination of the lower incisors, an action that can be expected due to the force applied by the device on the dentition. The other variables did not show statistically significant differences. Conclusion: After a mean time of 6.47 months of use of the mandibular advancement device, there were statistically significant changes in the dental positioning, but they were not clinically relevant. However, it is relevant that this device is commonly in use over long periods of time, making the monitoring of these patients of the utmost importance for the duration of their therapy.


Resumo: Introdução: Os aparelhos intraorais têm assumido cada vez mais um papel importante no tratamento da síndrome da apneia obstrutiva do sono, mas existem limitações a sua indicação e efeitos colaterais com o seu uso contínuo, assim como com o uso do aparelho de pressão aérea positiva contínua. Objetivos: Avaliar as alterações no posicionamento dentário produzido pelo uso contínuo do aparelho de projeção mandibular. Método: Através de estudo longitudinal prospectivo com amostra de 15 pacientes, com avaliação de documentações completas após um tempo médio de 6,47 meses do uso do aparelho oral de Twin Block para tratamento de pacientes com apneia, foram avaliadas as alterações do posicionamento dos dentes decorrentes do seu uso. As seguintes variáveis foram avaliadas: overjet, overbite, distâncias intermolares superior e inferior, distâncias intercaninos superior e inferior, índice de irregularidade de Little e ângulo do plano incisivo mandibular. Foi feito teste de correlação intraclasse e foram aceitos índices de correlação acima de 0,08. Após atestada a distribuição normal da amostra (Shapiro-Wilks), foi usado um teste paramétrico (teste t), com nível de significância de 5%. Resultados: Houve diminuição nos valores de overjet, overbite e irregularidade de Little e aumento nos valores da distância intercanino inferior e do ângulo do plano incisivo mandibular. Todas essas variáveis sofrem influência, com diferentes expressividades, da inclinação para frente dos incisivos inferiores, uma ação que pode ser esperada devido à força aplicada pelo aparelho sobre a dentição. As demais variáveis não demostraram diferenças estatisticamente significativas. Conclusão: Houve mudanças estatisticamente significativas no posicionamento dos dentes, porém clinicamente sem relevância, com um tempo médio de uso de 6,47 meses do aparelho de avanço mandibular. Contudo, deve-se considerar que o uso dessa aparelhagem é comum durante longos períodos, fazendo com que seja de suma importância o acompanhamento desses pacientes a longo prazo.

Abstract Introduction: The selection of appropriate sites for miniscrew insertion is critical for clinical success. Objectives: The aim of the present study was to evaluate how interradicular spaces measured on panoramic radiograph compare with Cone-Beam Computed Tomography (CBCT), and how crowding can influence the presence of available space for miniscrew insertion, in order to define a new "safe zones" map. Methods: A total of 80 pre-treatment panoramic radiographs and 80 CBCT scans with corresponding digital models were selected from the archives of the department of Dentistry, Aarhus University. Crowding was measured on digital models, while interradicular spaces mesial to the second molars were measured on panoramic radiographs and CBCTs. For panoramic radiographs, a magnification factor was calculated using tooth widths measured on digital models. Statistical analyses were performed to investigate the correlation between the amount of crowding and the available interradicular space. Visual maps showing the amount of interradicular spaces measured were drawn. Results: The most convenient interradicular spaces are those between the second molar and the first premolar in the mandible, and between the central incisors in the maxilla. However, some spaces were revealed to be influenced by crowding. Conclusions: Calibration of panoramic radiographs is of utmost importance. Generally, panoramic radiographs underestimate the available space. Preliminary assessment of miniscrew insertion feasibility and the related selection of required radiographs can be facilitated using the new "safe zone" maps presented in this article.


Resumo Introdução: a seleção de locais apropriados para a inserção de mini-implantes é crítica para o sucesso clínico. Objetivo: os objetivos do presente estudo foram avaliar as medições de espaços inter-radiculares feitas em radiografias panorâmicas e compará-las com as medições feitas com tomografia computadorizada de feixe cônico (TCFC), além de avaliar como o apinhamento pode influenciar na disponibilidade de espaços para inserção dos mini-implantes, no intuito de definir um novo mapa de "áreas seguras". Métodos: foram selecionadas, nos arquivos do departamento de Ortodontia da Aarhus University, 80 radiografias panorâmicas pré-tratamento e 80 imagens de TCFC com os modelos digitais correspondentes. O apinhamento foi medido nos modelos digitais, enquanto os espaços inter-radiculares mesiais aos segundos molares foram medidos nas radiografias panorâmicas e na TCFC. O fator de magnificação das radiografias panorâmicas foi calculado utilizando-se as larguras dentárias medidas nos modelos digitais. Análises estatísticas foram realizadas para investigar a correlação entre a quantidade de apinhamento e o espaço inter-radicular disponível. Foram desenhados mapas visuais mostrando a quantidade dos espaços inter-radiculares medidos. Resultados: os espaços inter-radiculares mais adequados são aqueles entre o segundo molar e o primeiro pré-molar inferior, e entre os incisivos centrais superiores. Porém, verificou-se que alguns espaços são influenciados pelo apinhamento. Conclusões: A calibração das radiografias panorâmicas é de suma importância, pois, geralmente, as radiografias panorâmicas subestimam o espaço disponível. A avaliação preliminar da viabilidade de inserção dos mini-implantes e a seleção das radiografias necessárias para isso podem ser facilitadas utilizando-se os novos mapas de "áreas seguras" aqui apresentados.

ABSTRACT Introduction: Plaster dental casts are routinely used during clinical practice to access maxillary dental arch form and assist on fabrication of individualized orthodontic archwires. Recently introduced, digital model technology may offer a limitation for the obtainment of a dental physical record. In this context, a tool for dental arch form assessment for chairside use is necessary when employing digital models. In this regard, paper print of the dental arch seems thus to be useful. Methods: In the present study, 37 lower arch models were used. Intercanine and intermolar widths and dental arch length measurements were performed and compared using plaster dental casts, digital models and paper print image of the models. Ortho Insight 3D scanner was employed for model digitalization. Results: No statistically significant differences were noted regarding the measurements performed on the plaster or digital models (p> 0.05). Paper print images, however, showed subestimated values for intercanine and intermolar widths and overestimated values for dental arch length. Despite being statistically significant (p< 0.001), the differences were considered clinically negligible. Conclusion: The present study suggests that paper print images obtained from digital models are clinically accurate and can be used as a tool for dental arch form assessment for fabrication of individualized orthodontic archwires.


RESUMO Introdução: os modelos de gesso são usados rotineiramente, durante a prática clínica, para avaliação da forma da arcada inferior e para auxiliar na confecção de arcos ortodônticos individualizados. A tecnologia dos modelos digitais, introduzida recentemente, pode oferecer uma limitação na obtenção de um registro físico da arcada dentária. Assim, quando se utilizam modelos digitais, faz-se necessária uma ferramenta clínica para obtenção da forma da arcada. Com essa finalidade, poderia-se imprimir, em papel, uma imagem da arcada dentária obtida a partir do modelo de gesso. Métodos: nesse estudo, 37 modelos da arcada inferior foram utilizados, nos quais foram realizadas medições das distâncias intercaninos, intermolares e comprimento da arcada; sendo, então, comparadas entre modelos de gesso, modelos digitalizados com um scanner Ortho Insight 3D e imagens impressas em folha de papel A4. Resultados: não foram encontradas diferenças estatisticamente significativas nas medidas realizadas nos modelos de gesso e modelos digitais (p > 0,05). As imagens impressas, contudo, mostraram valores subestimados para as distâncias intercaninos e intermolares, e superestimados para o comprimento da arcada. Apesar de serem estatisticamente significativas (p< 0,001), as diferenças foram consideradas clinicamente insignificantes. Conclusão: o presente estudo sugere que as imagens obtidas por meio dos modelos digitais e impressas em papel são clinicamente acuradas e podem ser utilizadas como uma ferramenta auxiliar na confecção dos arcos ortodônticos individualizados.

ABSTRACT Introduction: Etiology of dental crowding may be related to arch constriction in diverse dimensions, and an appropriate manipulation of arch perimeter by intervening in basal bone discrepancies cases, may be a key for crowding relief, especially when incisors movement is limited due to underlying pathology, periodontal issues or restrictions related to soft tissue profile. Objectives: This case report illustrates a 24-year old woman, with maxillary transverse deficiency, upper and lower arches crowding, Class II, division 1, subdivision right relationship, previous upper incisors traumatic episode and straight profile. A non-surgical and non-extraction treatment approach was feasible due to the miniscrew-assisted rapid palatal expansion technique (MARPE). Methods: The MARPE appliance consisted of a conventional Hyrax expander supported by four orthodontic miniscrews. A slow expansion protocol was adopted, with an overall of 40 days of activation and a 3-month retention period. Intrusive traction miniscrew-anchored mechanics were used for correcting the Class II subdivision relationship, managing lower arch perimeter and midline deviation before including the upper central incisors. Results: Post-treatment records show an intermolar width increase of 5 mm, bilateral Class I molar and canine relationships, upper and lower crowding resolution, coincident dental midlines and proper intercuspation. Conclusions: The MARPE is an effective treatment approach for managing arch-perimeter deficiencies related to maxillary transverse discrepancies in adult patients.


RESUMO Introdução: a etiologia do apinhamento dentário pode estar relacionada à constrição das arcadas dentárias em diversas dimensões, e a manipulação apropriada do perímetro da arcada, por meio da intervenção em casos de discrepâncias de base óssea, pode ser o fator chave para a dissolução do apinhamento, especialmente em casos onde a movimentação de incisivos é limitada em decorrência de problemas periodontais ou restrições relacionadas ao perfil facial. Objetivos: o presente relato de caso ilustra uma paciente de 24 anos de idade, com deficiência transversa de maxila, apinhamento das arcadas superior e inferior, má oclusão de Classe II, 1a divisão, subdivisão direita, incisivos superiores previamente traumatizados e perfil reto. A abordagem de tratamento não-cirúrgica e sem extrações foi viável devido à técnica de expansão rápida da maxila assistida por mini-implantes (MARPE, do inglês miniscrew-assisted rapid palatal expansion). Métodos: o dispositivo MARPE foi confeccionado a partir de um expansor Hyrax convencional apoiado em quatro mini-implantes. Foi adotado o protocolo de expansão lenta, com um período total de ativações de 40 dias e 3 meses de contenção. Uma mecânica de tração intrusiva apoiada em mini-implantes foi utilizada para a correção da relação de Classe II subdivisão direita, adequação do perímetro da arcada inferior e correção do desvio da linha média antes da inclusão dos incisivos centrais superiores. Resultados: os registros pós-tratamento demonstraram o aumento de 5,0 mm na distância intermolares, relação bilateral de Classe I de molares e caninos, resolução do apinhamento superior e inferior, linhas médias dentárias coincidentes e intercuspidação adequada. Conclusões: a técnica MARPE é uma abordagem de tratamento efetiva para a resolução da deficiência de perímetro das arcadas dentárias relacionada à discrepância maxilar transversa em pacientes adultos.

ABSTRACT Even though few technological advancements have occurred in Orthodontics recently, the search for more efficient treatments continues. This paper analyses how to accelerate and improve one of the most arduous phases of orthodontic treatment, i.e., correction of the curve of Spee. The leveling of a deep curve of Spee can happen simultaneously with the alignment phase through a method called Early Vertical Correction (EVC). This technique uses two cantilevers affixed to the initial flexible archwire. This paper describes the force system produced by EVC and how to control its side effects. The EVC can reduce treatment time in malocclusions with deep curves of Spee, by combining two phases of the therapy, which clinicians ordinarily pursue sequentially.


RESUMO Apesar de haver poucos desenvolvimentos tecnológicos nos últimos anos dentro da Ortodontia, a busca por tratamentos mais eficientes não cessou. Assim, o presente artigo visa analisar, de maneira lógica, como otimizar uma das fases do tratamento ortodôntico que mais demandam tempo: a correção da sobremordida exagerada. Pretende-se demonstrar como realizá-la concomitantemente ao alinhamento inicial, por meio de uma técnica denominada correção vertical precoce (CVP). Essa técnica utiliza dois cantilevers associados ao primeiro fio de alinhamento ortodôntico, mas não restritos a ele, a fim de iniciar a planificação da curva de Spee o mais cedo possível. Assim, o tempo de tratamento pode ser diminuído, já que duas fases do tratamento, normalmente realizadas de forma independente, podem ser realizadas ao mesmo tempo. Além disso, tanto o sistema de forças quanto os efeitos colaterais e os passos da execução da técnica serão devidamente apresentados e discutidos.

ABSTRACT Introduction: The aim of this multi-center retrospective study was to quantify the changes in alveolar bone height and thickness after using two different rapid palatal expansion (RPE) activation protocols, and to determine whether a more rapid rate of expansion is likely to cause more adverse effects, such as alveolar tipping, dental tipping, fenestration and dehiscence of anchorage teeth. Methods: The sample consisted of pre- and post-expansion records from 40 subjects (age 8-15 years) who underwent RPE using a 4-banded Hyrax appliance as part of their orthodontic treatment to correct posterior buccal crossbites. Subjects were divided into two groups according to their RPE activation rates (0.5 mm/day and 0.8 mm/day; n = 20 each group). Three-dimensional images for all included subjects were evaluated using Dolphin Imaging Software 11.7 Premium. Maxillary base width, buccal and palatal cortical bone thickness, alveolar bone height, and root angulation and length were measured. Significance of the changes in the measurements was evaluated using Wilcoxon signed-rank test and comparisons between groups were done using ANOVA. Significance was defined at p ≤ 0.05. Results: RPE activation rates of 0.5 mm per day (Group 1) and 0.8 mm per day (Group 2) caused significant increase in arch width following treatment; however, Group 2 showed greater increases compared to Group 1 (p < 0.01). Buccal alveolar height and width decreased significantly in both groups. Both treatment protocols resulted in significant increases in buccal-lingual angulation of teeth; however, Group 2 showed greater increases compared to Group 1 (p < 0.01). Conclusion: Both activation rates are associated with significant increase in intra-arch widths. However, 0.8 mm/day resulted in greater increases. The 0.8 mm/day activation rate also resulted in more increased dental tipping and decreased buccal alveolar bone thickness over 0.5 mm/day.


RESUMO Introdução: o objetivo do presente estudo retrospectivo multicêntrico foi quantificar as alterações na altura e na espessura do osso alveolar após o uso de dois diferentes protocolos de ativação na expansão rápida da maxila (ERM), bem como determinar se uma taxa de expansão mais rápida tem maior probabilidade de causar mais efeitos adversos, tais como inclinação dos dentes e do processo alveolar, fenestração e deiscência dos dentes de ancoragem. Métodos: a amostra consistiu de registros pré- e pós-expansão de 40 indivíduos (com idades entre 8 e 15 anos) que se submeteram à expansão rápida da maxila usando o aparelho Hyrax com quatro bandas como parte de seu tratamento ortodôntico para correção da mordida cruzada posterior. Os indivíduos foram divididos em dois grupos, de acordo com a taxa de ativação na expansão rápida da maxila (0,5 mm/dia e 0,8 mm/dia; n = 20 cada grupo). Imagens tridimensionais de todos os indivíduos da amostra foram avaliadas com o uso do software Dolphin Imaging v. 11.7 Premium. Foram analisadas as seguintes medidas: largura da base da maxila, espessura das corticais ósseas vestibular e lingual, altura do osso alveolar, angulação e comprimento das raízes. A significância das alterações nessas medidas foi avaliada com o teste não paramétrico de Wilcoxon e as comparações entre os grupos foram feitas usando a ANOVA. A significância foi definida como p ≤ 0,05. Resultados: as taxas de ativação na ERM de 0,5mm/dia (Grupo 1) e 0,8mm/dia (Grupo 2) causaram aumento significativo na largura da arcada após o tratamento. Porém, o Grupo 2 apresentou maior aumento, se comparado ao Grupo 1 (p < 0,01). A altura e a largura do osso alveolar vestibular diminuíram significativamente em ambos os grupos. Ambos os protocolos de tratamento resultaram em aumento significativo na inclinação vestibulolingual dos dentes. Porém, o Grupo 2 apresentou maior aumento, em comparação ao Grupo 1 (p < 0,01). Conclusão: ambas as taxas de ativação estão associadas a um aumento significativo nas larguras intra-arcada; entretanto, a taxa de 0,8mm/dia resultou em maior aumento. Essa mesma taxa de ativação também resultou em maior inclinação dos dentes e em redução mais acentuada da espessura do osso alveolar vestibular.

ABSTRACT

Introduction: The aim of this study was to assess changes in upper airway, dental arches dimensions and buccolingual inclinations of teeth in patients treated orthodontically with the Damon system. Material and Methods: Twenty-one patients of both sexes, mean initial age of 14.99 ± 1.97 years, with complete permanent dentition up to second molars, treated with non-extractions and minimum initial crowding of 4mm (maxillary: -6.71 ± 2,99 mm, mandibular: -5,12 ± 2,03 mm were taken to CBCT scan before (T1) and after (T2) orthodontic treatment with the Damon System. The volume, sagittal area and minimum axial area of the upper airway were measured in sagittal and axial sections by means of the upper airways analysis tool of Dolphin Imaging® software. Buccolingual inclinations were performed using CBCT-panoramic cross-sectionals based on the image that presented the most appropriate view of buccal face of the clinical crown. Arches dimensions were measured by means of 2D axial and coronal sections in 3D reconstructions. The reference points used were buccal and mesiobuccal cusp tips for the premolars and molars respectively. Distribution of the variables was verified by Shapiro-Wilk test. Changes in all variables from T1 to T2 were compared using paired t-test. The correlation between the upper airway changes with buccolingual inclinations of teeth and arches dimensions was verified by the Pearson correlation coefficient. Results: There was a significant increase in maxillary interfirst premolar, intersecond premolar, interfirst molar and intersecond molar width and the length with significant incisors and premolars labial inclinations. In the mandibular arch there was a significant increase of intercanine, interfirst premolar, intersecond premolar, inferfirst molar and intersecond molar width with significant buccal inclination at posttreatment, exception of the canine and the first molar of the left side. There was no significant increase in the length of the mandibular arch. There was no statistically significant difference in upper airway sagittal area, minimum axial area and volume changes after treatment. Conclusion: It can be concluded that Damon System produced a significant transversal increase in the posterior region of the arches with differences in teeth buccolingual inclinations at posttreatment. There were no significant differences in the upper airways; however, there was a significant negative correlation between the changes of length of mandibular arch and the sagittal area and volume airway.(AU)


Objetivo: Avaliar as alterações do volume e da área das vias aéreas superiores, as dimensões transversais e o comprimento dos arcos dentários e as inclinações dentoalveolares em pacientes tratados ortodonticamente com o sistema Damon. Material e métodos: A amostra foi constituída de tomografias computadorizadas cone beam (TCCB) iniciais (T1) e finais (T2) de 21 pacientes com idade inicial média de 14,99 anos (DP 1,97), má oclusão de Classe I de Angle, apinhamento mínimo de 4mm (superior: -6,71mm (DP 2,99), inferior: -5,12mm (DP 2,03), dentadura completa até segundos molares, tratados ortodonticamente sem extrações com braquetes autoligáveis do sistema Damon. As tomografias computadorizadas foram importadas no formato DICOM para o programa Dolphin Imaging® para análise das varáveis. O volume, área e área axial mínima das vias aéreas superiores foram mesuradas nos cortes sagital e axial por meio da ferramenta upper airways analysis do software Dolphin Imaging®. Para mensuração das inclinações dentárias, foram utilizados cortes parassagitais gerados a partir da imagem panorâmica da TCFC. As medidas foram realizadas a partir do ângulo formado pela linha vestibular da coroa clínica (LVCC) e o bordo superior ou inferior da imagem selecionada. As dimensões transversais dos arcos foram mensuradas por meio de cortes axiais das reconstruções 3D. Para os premolares, foram utilizadas como referência as pontas de cúspide vestibular e para os molares as pontas de cúspide mesiovestibular. As variáveis foram submetidas ao teste de Shapiro-Wilk para testar sua distribuição. As alterações de todas as variáveis de T1 para T2 foram comparadas por meio dos testes t-pareado. A correlação entre as alterações das vias aéreas superiores com os possíveis fatores associados foi verificada pelo coeficiente de correlação de Pearson. Resultados: Houve um aumento significativo das distâncias transversais 4-4 / 5-5 / 6-6 / 7-7 e do comprimento do arco superior com inclinações vestibulares significativas dos incisivos e pré-molares. No arco inferior houve um aumento significativo das distâncias transversais 3-3 / 4-4 / 5-5 / 6-6 / 7-7 sem inclinação vestibular significativa apenas do canino e do primeiro molar do lado esquerdo. Não houve aumento significativo do comprimento do arco inferior. Não houve diferença estatisticamente significante da área, área axial mínima e volume das vias aéreas pós-tratamento. Conclusão: Pode-se concluir que o Sistema Damon produziu um aumento transversal significativo na região posterior dos arcos com diferenças nas inclinações dentárias. Não houve diferenças significativas nas vias aéreas superiores, entretanto, houve significante correlação negativa entre as alterações comprimento do arco inferior com a área e o volume das vias aéreas.(AU)

ABSTRACT

Purpose: The aim of this study was to evaluate the occlusal and arch form stability of self-ligating Damon System appliance treatment, after a mean period of 3 years. Methods: The sample consisted of 20 patients (10 female and 10 male) Class I malocclusion with mild to moderate crowding treated orthodontically without extractions, with a mean initial age of 14.66 years (SD +1.40), mean final age of 17.70 years (SD +1.39) and mean age at posttreatment of 20.62 years (SD +1.42). The mean treatment time was 3.03 years (S.D. +1.17) and the mean posttreatment time was 2.92 years (S.D +0.88). Initial, final and posttreatment dental casts and panoramic radiographs of each patient were evaluated. Crowding, transverse and anteroposterior arch dimensions were measured in digital models in the maxillary and mandibular dental arches. For evaluation of the occlusal stability of orthodontic treatment it was used the Objective Grading System (OGS) in dental casts and panoramic radiographs. Intragroup comparison of all variables evaluated was performed by ANOVA and Tukey test, when necessary. For OGS comparison the dependent T test were used. Results: During treatment, there was significant decrease in the Little's Irregularity Index in maxillary and mandibular arches. No statistically significant changes in the Little's Irregularity Index were found during the follow-up period after debonding in a mean period of 2.9 years. There was significant increase in all measurements of the maxillary arch width and arch length with treatment. The mandibular measurements of arch width and arch length also showed a significant increase during treatment. At posttreatment, all maxillary and mandibular measurements remained stable, with no significant relapse of arch changes. The OGS index did not show significant changes during the posttreatment period. Conclusions: Treatment with self-ligating Damon System appliances showed increase in maxillary and mandibular arch widths and lenghts, which remain stable after a mean period of 3 years posttreatment. Treatment with this system provides maxillary and mandibular incisor alignment, as defined by Little's Irregularity index, and occlusal stability, as defined by the OGS score, with stability after a mean period of 3 years posttreatment.(AU)


Objetivo: O objetivo deste estudo foi avaliar a estabilidade oclusal e a estabilidade da forma do arco em pacientes tratados com aparelho autoligável do Sistema Damon. Material e métodos: A amostra foi constituída por radiografias panorâmicas e modelos de gesso iniciais (T1), finais (T2) e pós-tratamento (T3) de 20 pacientes (10 homens e 10 mulheres) com má oclusão de Classe I, apinhamento moderado a severo, tratados ortodonticamente sem extrações. Os pacientes apresentaram idade inicial média de 14,66 anos (D.P= +1,40), idade final média de 17,70 anos (D.P= +1,39), tempo médio de tratamento 3,03 anos (D.P= +1,17), idade de pós-tratamento média de 20,62 anos (D.P= +1,42) e tempo médio de pós tratamento de 2,92 anos (D.P= +0,88). O Índice de Estabilidade Oclusal (OGS), foi avaliado por meio de modelos de gesso e radiografias panorâmicas. As medidas transversais, sagitais e o Índice de irregularidade de Little foram realizadas utilizando-se modelos de gesso que foram digitalizados com o scanner 3Shape R700 3D (3Shape A/S, Copenhagen, Dinamarca) e mensurados através do Software OrthoAnalyzerTM 3D (3Shape A / S, Copenhagen, Dinamarca). Para comparação intragrupo de todas as variáveis avaliadas entre as fases inicial (T1), final (T2) e controle (T3), ANOVA e teste de Tukey foram utilizados. Para avalição do OGS foi utilizado o teste t dependente. Resultados: No arco superior todas as medidas da forma e comprimento do arco apresentaram um aumento com o tratamento (T2-T1), com exceção do Índice de Irregularidade de Little que diminuiu significantemente com o tratamento. As medidas mandibulares da forma e comprimento do arco também tiveram um aumento significante com o tratamento, já o Índice de Irregularidade de Little diminuiu significantemente. No período pós-tratamento (T3-T2) todas as medidas maxilares e mandibulares permaneceram estáveis, não apresentando recidiva significante da forma do arco e do Índice de Irregularidade de Little. O índice OGS não apresentou alterações significantes do fim do tratamento para o controle. Conclusão: O tratamento com aparelho autoligável do Sistema Damon apresentou aumentos na forma e comprimento do arco, mantendo-se estáveis após o período de contenção. O tratamento com este Sistema promoveu o alinhamento dos incisivos tanto superiores quanto inferiores, com resultados oclusais estáveis após um período médio de 3 anos pós-tratamento.(AU)

ABSTRACT

Abstract Carotid artery disease has been linked with cerebral vascular accident, also known as stroke, cerebral hemorrhage, or cerebral ischemia. It is caused by narrowing or obstruction of arteries in the neck (the carotid arteries) that are responsible for transporting blood from the aorta to the brain. Panoramic radiographs are used in dentistry to show both dental arches as a supplement to the clinical dental examination. The objective of this study is to highlight the importance of panoramic radiographs for diagnosis of arterial disease, by means of a bibliographic review. The PubMed database was searched using the keywords “atherosclerosis” and “panoramic”, with the filters “last 5 years” and “humans”. Twenty articles were identified, six of which were chosen for this study because they were open access. The review concluded that panoramic radiographs enable early diagnosis of carotid artery calcification, resulting in earlier interventions, and offer an accessible cost.


Resumo Doenças das artérias carótidas estão associadas a casos de acidente vascular cerebral, também chamados de derrame ou isquemia cerebral. Ocorrem devido ao estreitamento ou obstrução das artérias do pescoço, ou artérias carótidas, responsáveis por levar o sangue da aorta para o cérebro. A radiografia panorâmica é um exame de ambos os arcos dentários que ajuda no exame clínico odontológico. Destacar a importância da radiografia panorâmica no diagnóstico de doença arterial é o objetivo deste trabalho, que foi realizado através de pesquisa bibliográfica na PubMed, com as palavras-chave “atherosclerosis” e “panoramic”, e os filtros: últimos 5 anos e humanos. Vinte artigos foram elencados, dos quais seis formaram a base para este estudo, graças ao acesso livre ao seu conteúdo. Conclui-se que a radiografia panorâmica permite o diagnóstico precoce de calcificação da artéria carótida, levando a uma intervenção precoce a um custo acessível.

Objective: The present study aimed at evaluating the reliability of Bolton analysis in tridimensional virtual models, comparing it with the manual method carried out with dental casts.Methods:The present investigation was performed using 56 pairs of dental casts produced from the dental arches of patients in perfect conditions and randomly selected from Universidade Federal da Bahia, School of Dentistry, Orthodontics Postgraduate Program. Manual measurements were obtained with the aid of a digital Cen-Tech 4"(r) caliper (Harpor Freight Tools, Calabasas, CA, USA). Subsequently, samples were digitized on 3Shape(r)R-700T scanner (Copenhagen, Denmark) and digital measures were obtained by Ortho Analyzer software.Results:Data were subject to statistical analysis and results revealed that there were no statistically significant differences between measurements with p-values equal to p = 0.173 and p= 0.239 for total and anterior proportions, respectively.Conclusion:Based on these findings, it is possible to deduce that Bolton analysis performed on tridimensional virtual models is as reliable as measurements obtained from dental casts with satisfactory agreement.


Objetivo: o presente estudo teve como objetivo avaliar a confiabilidade da análise de Bolton em modelos virtuais tridimensionais, comparando-a com a realizada em modelos de gesso.Métodos:foram utilizados 56 pares de modelos de gesso das arcadas dentárias de pacientes oriundos do Curso de Especialização em Ortodontia da Faculdade de Odontologia da Universidade Federal da Bahia, escolhidos aleatoriamente e em perfeito estado. Medidas manuais foram obtidas utilizando-se o paquímetro digital Cen-Tech(r) 4" (Harpor Freight Tools, Calabasas, CA, EUA). Em seguida, os mesmos foram digitalizados pelo scanner R-700TM(3Shape(r), Copenhagen, Dinamarca) e, por meio do programa Ortho AnalyzerTM, da mesma marca, foram obtidas as medidas digitais.Resultados:os dados foram submetidos a testes estatísticos e os resultados demonstraram que não houve diferença estatisticamente significativa nos dois tipos de medições com valores de p = 0,173 e p = 0,239, respectivamente, para as proporções total e anterior.Conclusão:com base nesses achados, pode-se inferir que a análise de Bolton realizada em modelos virtuais tridimensionais é tão confiável quanto a obtida em modelos de gesso, apresentando uma concordância satisfatória.

OBJECTIVE: The aim of the present study was to compare dentoalveolar changes in mandibular arch, regarding transversal measures and buccal bone thickness, in patients undergoing the initial phase of orthodontic treatment with self-ligating or conventional bracket systems. METHODS: A sample of 25 patients requiring orthodontic treatment was assessed based on the bracket type. Group 1 comprised 13 patients bonded with 0.022-in self-ligating brackets (SLB). Group 2 included 12 patients bonded with 0.022-in conventional brackets (CLB). Cone-beam computed tomography (CBCT) scans and a 3D program (Dolphin) assessed changes in transversal width of buccal bone (TWBB) and buccal bone thickness (BBT) before (T1) and 7 months after treatment onset (T2). Measurements on dental casts were performed using a digital caliper. Differences between and within groups were analyzed by Student's t-test; Pearson correlation coefficient was also calculated. RESULTS: Significant mandibular expansion was observed for both groups; however, no significant differences were found between groups. There was significant decrease in mandibular buccal bone thickness and transversal width of buccal bone in both groups. There was no significant correlation between buccal bone thickness and dental arch expansion. CONCLUSIONS: There were no significant differences between self-ligating brackets and conventional brackets systems regarding mandibular arch expansion and changes in buccal bone thickness or transversal width of buccal bone. .


OBJETIVO: o objetivo do presente estudo foi comparar as alterações dentoalveolares transversais e a espessura óssea da arcada inferior em pacientes submetidos ao tratamento ortodôntico utilizando sistemas de braquetes autoligáveis ou convencionais. MÉTODOS: uma amostra de 25 pacientes requerendo tratamento ortodôntico foi recrutada com base no tipo de braquete. No Grupo 1, 13 pacientes foram tratados com braquetes autoligáveis (SLB, slot 0,022"); o Grupo 2 incluiu 12 pacientes, nos quais foram colados braquetes convencionais (CLB, slot 0,022"). Utilizou-se tomografia computadorizada de feixe cônico e um programa 3D (Dolphin) para avaliar as alterações pré-tratamento (T1) e 7 meses após o início desse (T2). As medições em modelos de gesso foram realizadas com o auxílio de um paquímetro digital. As diferenças intergrupos, bem como intragrupo, foram analisadas por meio de teste t de Student. Além disso, o coeficiente de correlação de Pearson foi utilizado. RESULTADOS: alterações dentoalveolares significativas foram observadas em ambos os grupos. Entretanto, não houve diferenças significativas entre os grupos. Houve uma diminuição da espessura óssea na região posterior e das medidas transversais em ambos os grupos. Não houve uma correlação significativa entre a espessura óssea posterior e a expansão da arcada dentária, em nenhum dos dois sistemas de braquetes utilizados. CONCLUSÕES: comparando-se o uso dos aparelhos autoligáveis e convencionais, concluiu-se que não houve diferenças dentoalveolares significativas quanto à expansão da arcada inferior e quanto à espessura óssea posterior. .

Background: Lingual vascular canal (LVC) is an important anatomical structure in mandibular anterior region. Trauma to this structure during implant placement has been reported in this study. Dental computed tomography (DCT) provides a three-dimensional visualization of lingual vascular canal. Aim: To assess the frequency, location, and size of LVC using dental CT. Materials and Methods: A retrospective analysis of 75 mandibular DCT was done. Evaluation was done to detect the frequency, size, and number of lingual vascular canal using Seimens Somatom Sensation 64. Results: About 73.3% patients (male=34, female=21) demonstrated presence of LVC with only one patient showing two canals. The mean distance from the inferior border of mandible was 0.5 mm, S.D.±0.70. The mean diameter of the canal was 0.31 mm, S.D.±0.70. Conclusion:Dental CT provides adequate information regarding frequency, number, and size of lingual vascular canal, which is an important anatomical structure in mandibular anterior region.

Background and Objective: Digital photo stimulable phosphor (PSP)-based radiography has many known theoretical advantages over conventional film radiography but its diagnostic efficacy has to be assessed clinically. This study compared the efficiency of conventional film-based panoramic radiographs with that of digital PSP-based panoramic radiographs in the assessment of position and morphology of impacted mandibular third molars. Materials and Methods: We selected a total of 80 impacted mandibular third molars that fulfilled the inclusion and exclusion criteria of this study. Both conventional film-based panoramic radiographs and digital PSP-based panoramic radiographs were taken of all the study samples and the teeth were later surgically removed. Conventional film-based and digital PSP-based panoramic radiographs were compared for their relative efficiencies in the assessment of impaction status, position of tooth, number of roots, root morphology, and proximity to mandibular canal of impacted mandibular third molars. An oral surgeon graded these same factors during/after surgical exploration and this assessment was considered the gold standard. The data obtained were statistically analyzed using descriptive statistics, chi-square test, and McNemar's test. Results: There was no statistically significant difference between conventional film-based radiographic assessment and digital PSP-based panoramic radiographic assessment of impaction status, position of tooth, number of roots, and proximity to mandibular canal of impacted mandibular third molars (P>0.05). However, there was significant difference between the two methods with regard to assessment of root morphology of impacted mandibular third molars (P=0.00). Interpretation and Conclusion : Conventional film-based panoramic radiographs and digital PSP-based panoramic radiographs were comparable in their accuracy in the preoperative study of impacted mandibular third molar with regard to impaction status, tooth position, number of roots, and proximity to the mandibular canal. Digital PSP-based panoramic radiographs were more accurate than conventional film-based panoramic radiographs in the assessment of root morphology of impacted mandibular third molars. Hence, we conclude that digital PSP-based panoramic radiographs can be used as an effective alternative to conventional film-based panoramic radiographs for assessment of position and morphology of impacted mandibular third molars.

ABSTRACT

Aim: The purpose of this study was to determine the correlation between pulp stones and cardiovascular disorders, Type II diabetes mellitus, autoimmune disorders and dental wear defects. This study also aims to evaluate the frequency of pulp stone in population of Dakshina Kannada district (Karnataka, India) and to determine association of pulp stones in different sexes, tooth type, dental arches and sides. Patients and Methods: A total of 1432 teeth of five groups were examined, comprising of patients with C.V.S. disorders; Type II diabetes mellitus, autoimmune disorders, dental wear defects and control group. Teeth were examined under 2Χ magnification on radio visiograph (RVG) and conventional intra-oral periapical radiograph. The presence or absence of pulp stones were recorded. Statistical Analysis Used: Chi-square analysis was used to compare the frequency of occurrence of pulp stones between sexes, tooth type, dental arches and sides. Results: Pulp stones were found in 134 (9.35%) of 1432 teeth detected. Significantly, higher numbers of pulp stones were recorded in patients with cardiovascular disorder (15.86%) than other groups. The occurrence of pulp stones were significantly higher in molars (18.29%) than premolars (6.6%) and in maxillary arch (12.36%) than in mandibular arch (5.95%). No significant difference was found between sexes and sides. Conclusion: Positive correlation was found between systemic disorder and pulp stones. Cardiovascular patients had maximum number of pulp stones followed by dental-wear defects and least number of pulp stones were evident in control group.

ABSTRACT

Background: Information on the position of the mental foramen is important for dental surgeons. Variations in its position can be a cause of complications during local anesthesia or surgical procedures. The usual position of the mental foramen in an Iranian population has not been previously reported. Aims: The purpose of this study was to determine the most common location of the mental foramen in an Iranian population. We also analyzed gender differences and the symmetry of location within individuals. Materials and Methods : 400 panoramic radiographs were evaluated with regard to the location and symmetry of the mental foramina in male and female subjects. Results : We found that the mental foramen was located between the first and second premolars in 47.2% of patients and in line with the second premolar in 46%. In 49.2% of males, the mental foramen was in line with the second premolar. In 50.9% of females it was between the first and second premolars. It was symmetrical in 85.7%. Conclusions : Based on this study it appears that the most common position of mental foramen is either between the two premolars or in line with the second premolar. This is in concordance with previous studies.

ABSTRACT

In the recent World Health Organization classification of odontogenic tumours, desmoplastic ameloblastoma has been characterized as a variant of ameloblastoma, with specific clinical, radiographical, and histological features. Till date, 145 cases have been reported in Japanese, Chinese, Malaysian, Western, and African populations, with very few cases described in Indians. Here, we report five cases in the Indian population. The male to female ratio was 3:2. The mean age at diagnosis was 33.2 years. Four of the tumours were located in the maxilla, in the anterior premolar region. The lone mandibular tumour was located anteriorly, crossing the midline. Three of the tumours had a mixed radiologic appearance with poorly defined borders. Unilocular change was seen in one of them. Two tumours presented as unilocular radiolucencies with specks of radiopacities and well-circumscribed borders. Histologically, irregular odontogenic islands, with a stretched-out 'kite-tail' appearance, were seen in a dense desmoplastic stroma. The peripheral layer of the epithelial islands was made up of flattened cells and the inner core was made up of spindle-shaped and, in some instances, squamous-shaped cells. In two cases, odontogenic epithelium in the form of follicles, typical of solid/multicystic ameloblastoma, was seen and these were typed as 'hybrid' variants. All the cases were treated by resection.
